Job Description

SUMMARY: 

Provides individualized assistance to patients, families and caregivers to help overcome healthcare system barriers and to facilitate timely access to quality medical and psychosocial care throughout the continuum of cancer care.

Under general administrative direction, coordinates referrals from physicians and health care facilities for patients; provides a significant level of patient education related to their illness and planned treatment

Improves overall outcomes by reducing delays between abnormal testing, diagnosis and treatment; and reducing patient and family anxiety.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Patient Coordination/Advocate - works closely with physicians in all specialty areas to coordinate patient’s care plan communication; works with multidisciplinary team to maintain and implement up-to-date coordinated care plan; communicate wit all members of the healthcare t3eam on behalf of the patient 
  • Patient Referral – Uses Cancer Center protocols to refer patients to the appropriate specialist for diagnosis and/or treatment and assists patients and their families in obtaining referrals to needed specialists to include making appointments in a timely manner.  Provides support and counseling, as appropriate to the clinical situation. 
  • Clinical Process Development – Works with medical oncologists to develop and maintain clinic processes and pathways. 
  • Optimizes Outcomes – Proactively develops and enhances processes with the goal of improving the clinical and persona experience for referred patients and efficiency for both the referring and accepting physician.  Reduce the time span from diagnosis to definitive treatment therefore reducing anxiety and fear.  Reduce obstacles that would impede care. 
  • Liaison – Communicates with referring physician’s offices as appropriate to the patient and physician’s needs.  Investigates, resolves and documents the resolution of patient complaints concerning services provided by the Cancer Center. 
  • Education – Works with Community Relations to educate referring physicians, facilities and the general public on available services and role of the nurse navigator. 
  • As needed – Performs various duties as needed to successfully fulfill the function of the position and The Cancer Center’s success.

Job Requirements

QUALIFICATION REQUIREMENTS:

Case Manager LCSW with 10+ yrs. experience or Registered Nurse with 4-5 years clinical experience required.

PHYSICAL DEMANDS AND WORKING CONDITIONS:

• Medium Work as defined by the U.S. Department of Labor constitutes a maximum lift of 21-50 pounds on occasion and/or a maximum lift of 11-25 pounds on a frequent basis

• May be required to assist in the turning, lifting, or moving patients as well as

walking about from patients’ rooms, as well as to and from various hospital areas
