Job Description

SUMMARY:  The licensed practical nurse provides nursing care according to the physician’s orders and in conformance with the ANA Standards of Practice and Care, ANA Code of Ethics, unit specific standards, patient care policies and procedures, unit specific policies and procedures, and the Louisiana Nurse Practice Act. 

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • The Licensed Practical Nurse works under the direction of the Registered Nurse
  • Obtains information to contribute to an accurate and current patient assessment, patient care plan, and discharge plan
  • Delivers compassionate care to patients and supports members of the patient’s family
  • Includes the patient and caregivers in this process to obtain accurate information/history and contributes to the RN’s assessment
  • Evaluates the patient’s needs, reports findings in a timely manner, evaluates the patient frequently to determine changes in conditions/needs, and performs basic nursing monitoring skills, as well as contributes to a current patient assessment – I&O, IV, VS
  • Gathers information appropriately using sound nursing assessment skills, proper interviewing techniques, and collaborates with the multi-disciplinary team in discharge planning
  • Demonstrates competency and knowledge of current standards of practice
  • Accurately performs current practice standards and performs basic nursing skills in a competent manner
  • Administers medication correctly and carries out orders promptly
  • Is responsible for accurately documenting patient care in a timely manner being attentive to detail when documenting procedures, medications, IV maintenance, and abnormal findings
  • Should demonstrate knowledge of disease process, recognizes abnormal conditions, and reports valid patient care conditions and changes
  • Provides patient care in an organized manner, utilizing knowledge of nursing process – assessments, planning, interventions, and evaluations
  • Under the direction of the RN, the LPN will interpret the information needed to identify each patient’s requirements relative to the age-specific needs
  • Provides individualized health education to the patient and caregivers to specifically meet the patient’s needs related to the current diagnoses
  • The education process is from admission through discharge and includes all age groups pertaining to the patient population served
  • Provides care in a safe and therapeutic environment

    • Initiates patient contact and prepares the patient for visit by indicating chief complaint and reason for visit,

    obtaining vital signs and obtaining patient intake information.

    • Includes the patient and caregivers in the intake process to obtain accurate information/history.

    • Provides care in an organized manner, utilizing knowledge of the nursing process.

    • Demonstrates competency and knowledge of current standards of practice.

    • Manages patient calls/messages, medication refills, and test results to ensure continuity of care.

    • Cleans exam room after each patient and prepares room for the next patient.

    EMR.

    • Work with patients to assist them in developing self-management skills in an effort to change

    behaviors and improve outcomes

    • Collaborate with patient care team, promptly communicating any information that is important for

    continuity of care

    • Provides culturally appropriate care to all patients and work to address or remove barriers when

    discovered

    • Reinforces discharge and follow-up instructions.

    • Will interpret the information needed to identify each patient’s requirements relative to age-specific

    needs

    • Assist physician in performing basic life support and other emergency measures.

    • Performs various specimen collections including but not limited to venipuncture and in-house

    laboratory testing, and daily quality checks.

    • Set up and assist physician or NP with minor office procedures including proper sterile field as

    indicated.

    • Maintain clinical supply PAR levels and place orders as needed.

    • Assists with scheduling ancillary services and assuring that the pre-certification process is complete

    prior to scheduling as needed.

    • Coordinates patient’s care by scheduling referrals with other physician offices in a timely manner,

    forwards patient records and documents the same in EHR.

    • Works together with the Quality Nurse Analyst, physicians/providers, physician service leaders,

    clinical service leader, and other staff to assess patient population statistics, develop appropriate

    interventions and improve population health outcomes.

    • Participates in daily huddles, staff meetings and in evaluations of performance and job satisfaction

    • Adheres to all requirements, policies, procedures and mandatory training as identified by LGH/UHC.

    • Ensures patient confidentiality and privacy.

    • Adheres to OSHA and Infection Control Policies

Job Requirements

EDUCATION/TRAINING/EXPERIENCE:

  • Maintains current licensure as Licensed Practical Nurse in the State of Louisiana
  • CPR Certification
  • IV therapy

PERFORMANCE REQUIREMENTS:

  • Must comply with all Standards of Behavior
  • Adhere to all requirements, policies, procedures and mandatory training as identified by LGMC.

EQUIPMENT OPERATED:

  • Computer
  • Various types of medical instruments & equipment

PHYSICAL DEMANDS AND WORKING CONDITIONS:

  •  Medium Work as defined by the U.S. Department of Labor constitutes a maximum lift of 21-50 pounds on occasion and/or a maximum lift of 11-25 pounds on a frequent basis
  • May be required to assist in the turning, lifting, or moving patients as well as

      walking about from patients’ rooms, as well as to and from various hospital areas
